Getting to know your needs

All services are preceded by a detailed domiciliary care needs assessment, for funded clients this will be completed by the funding body. When we are approached to provide this service the first action is to verify that we are fully able to provide the level and intensity of care needed.

On satisfactory completion of the assessment/verification and before the service begins details for the provision of service will be agreed (as appropriate) between ourselves, the service user, family/representative and funding body.

A risk assessment will also be carried out for all services that are to be provided and a service user plan produced which will define, in detail, the nature and extent of care which we undertake to provide. As appropriate, this plan will be produced in consultation with senior care co-ordinator, the service user, family/representative and funding body.

The plan is formally reviewed on a 3-6 monthly basis depending on the needs of the service user. However, service users are continually under observation and the plan is updated if any change in circumstances occurs.
